Those participating in an annual golf tournament may also use this form to pay their registration fees using a credit card. Just enter "Golf Tournament" in the "Reason for Donation" field along with the name of the applicable item(s) being paid for. Individuals who are part of a Team or Sponsorship may find it easier to settle their share with the Captain or Leader offline. We appreciate your supporting us via the golf tournament.
