We appreciate your participation in our Annual Golf Tournament and wish you the best of luck - like, a nice, sunny day and a hole-in-one! For non-golfers, enjoy your lunch and good luck with the silent auction!
Your participation will help us to achieve our mission of providing services that promote the health, hygiene and dignity of persons experiencing homelessness and poverty in Southeast Houston, Texas.

Gold - $1,610 Silver - $930 Team - $680 Hole - $250 Individual - $170 Lunch only - $45
1. If you wish to pay via credit card, please use the entry form provided below. You may also download a copy of the brochure at 'here' below (#4) and fill in the required information by hand. Send that with your check to the Crosroads at Park Place Address, attention Doug Fortner. Please do not use the credit card portion of the brochure. Your payment guarantees you or your team a spot in the tournament. You may also register and make payment the day of the tournament.
2. On the entry form below, Team Captains may pay for their entire team with teammates settling their share with their Captain or Sponsor offline. Of course, each individual golfer or those participating for e.g., just lunch only, may pay just for themselves.
3. After entering your fee and agreeing to pay the small, extra credit card processing fee to help us out, please fill out the short participant form at the bottom to share your team members' contact information. Then, hit the "Submit Payment & Contact Info" button.
